Grade: G4: £25,183 - £27,269 per annum
Community Reablement Co-ordinators: This is a pivotal role within the service as you will be the conduit between the practitioners, care provider, and residents. Great IT and communication skills are a must.

About the role
As a Community Reablement Co-ordinator, you will play a vital role through brokering services, communicating with external providers, colleagues, and residents to deliver and maintain a high standard in service provision. Utilising your IT skills will ensure information is accurately recorded on the appropriate systems relating to purchasing of services.
