Are you an advanced clinical pharmacist practitioner looking to expand your knowledge and skills and try something different? Are you a champion of medicines safety and patient centred care? We have an exciting opportunity for a dynamic and motivated pharmacist to join a diverse multidisciplinary team as part of our urgent community response and reablement teams.
We are looking for an experienced clinical pharmacist who has experience of influencing within teams and across organisations. The post holder should be enthusiastic and patient focused with a keen interest in frailty and shared decision making. The role offers flexibility to plan and manage a patient caseload and lead a team of pharmacists and pharmacy technicians to meet the needs and goals of the service. The role will involve regular travel across the local area within Lambeth and Southwark; the post holder must be able to meet the travel requirements of this post.

Main duties of the job
Many of our patients require multidisciplinary interventions. The post holder will be responsible for leading the clinical pharmacy service, ensuring that the use of medicines is optimised through safe prescribing, handling and administration processes. They will also undertake domiciliary medication reviews and co-ordinate the implementation of pharmaceutical care plans to ensure that patients get the best outcomes and experience from taking medicines.

Integrated Local Services provide a range of community and inpatient services for the communities of Lambeth and Southwark. Services provided include neighbourhood nursing, specialist rehabilitation & reablement, specialist palliative & end of life care, vulnerable adults and prevention services, and the @Home service. Services are delivered in various locations e.g. health centres, community bed-based units, and patients’ homes. We work closely with teams at King's College Hospital and with social, mental health and voluntary sectors. Our aim is to enable people to live healthy independent lives for as long as possible within their normal place of residence.

Experience
* Working across the wider health and social care integrated care settings or partnership working within multidisciplinary teams
* Developing and undertaking audits
* Writing guidelines, standard operating procedures and policies
* Delivering training
* Line management, mentoring or coaching experience
* Primary care experience
Knowledge
* Understands policies and the context around managing long term conditions, case management and reducing unnecessary hospital admissions in the NHS and their implication for medicines management
* Understands primary and secondary care working and organisational structures.
* Sound therapeutic and clinical knowledge in care of older people
* Understands pharmacy and medicines management issues in primary/community and secondary care that affect patient care
* Understands the wider issues in the community and across the interface with social care that affect medicines optimisation particularly during transition of care
Skills
* Ability to provide expert advice and communicate complex pharmacy issues that may be frequently challenged
* Good clinical skills including evidence-based practice
* Able to provide leadership in complex circumstances and gain support for medicines and pharmacy issues with stakeholders
* Ability to empathise with patients and motivate them to modify their behaviour to improve medicines use
* Ability to effect change within own service and facilitate change within the organisation
* Proven ability to work as part of a multidisciplinary / organisational teams
* Proven ability to establish, develop and maintain effective working relationships across disciplines and organisations
* Ability to spread ideas and good practice to other members of healthcare teams
Qualifications
* Postgraduate Certificate, Diploma or MSc in pharmacy practice
* Substantial evidence of CPD and continuing education in pharmacotherapy of older people and long term conditions
* Qualified nonmedical prescriber
* Specialist training, qualification or education in the care of older people
* Qualified advanced clinical practitioner
