As an integral and highly valued member of the multi- disciplinary team, the Peer Support Worker will provide formalised peer support and practical assistance to service users in order for them to regain control over their lives and their own unique recovery process.
Through sharing the wisdom from own lived experience, inspire hope and believe that recovery and the principles of recovery are possible. Within a relationship of mutuality, facilitate and support information sharing to promote choice, self-determination and opportunities for the fulfilment of socially valued roles and connection to local communities.
The Peer Support Worker will take a lead role in embedding recovery values within the service setting in which they work, alongside other Trust Recovery Champions, and act as an ambassador for if /recovery for the Trust with external agencies and partner organisations.
The Peer Support Worker will promote their role, through the provision of information and a range of learning opportunities for the multi-disciplinary team and others across the Trust.
Report directly to the Social Work Manager the Peer Support Worker will be responsible for the delivery of peer support interventions as agreed within the peer relationship and feeding into the CPA process

• To coordinate a percentage of Local Authority statutory reviews as commissioned via CPFT
• To support the user to meet the outcomes they have identified within their statutory assessment of needs through their personal support plan, by drawing on your mutual resources as peers and utilising a range of recovery tools, techniques and experience.
• To ensure that the personal, social, cultural and spiritual strengths are recognised.
• To develop personalised goals in collaboration with the individual their Social Worker/ Care Coordinator which are specific, measurable, achievable and outcome focused.
• To work with people to facilitate access and engagement in a range of socially inclusive and vocational community-based facilities.
• Model personal responsibility, self-awareness, self-belief, self advocacy and hopefulness via the telling of own recovery story (where appropriate) to inspire and instil confidence in peers.
• To work with people to facilitate the development of appropriate professional relationships based on respect and promote hope. Help people gain access to resources including benefits and welfare rights, employment and legal rights by offering practical support, advice and information.
• Help to identify early signs of relapse by monitoring the person’s progress, level of functioning and mental state and alert the appropriate staff involved in the individual’s care.
• Share/teach coping, self help and self-management techniques (where appropriate) within the peer relationship.
• To support service users to identify and overcome fears and within a relationship of empathy, trust and honesty, challenge negative self-talk.
• Participate in regular planning and review meetings with the social work manager and other appropriate professionals
• To positively develop links with other support service within CPFT, PCC and Voluntary sector organisations, (Recovery Coach, Community Connectors, Peer Support Workers, Volunteers, - Recovery College)
• Maintain adequate records as required by existing procedures, entering appropriate details on the person’s case notes as necessary.
• Attend and participate in wider team/care plan review meetings as appropriate.
• To complete 1:1 work with the service user to enable and re-able, eg: accompany to appointments/meetings of their choice and perform a range of community activities to maximise independence, confidence and social involvement.
• Have a focus on the rights of service users and carers at all times.
• To develop and maintain good working relationships with statutory, independent and voluntary agencies to support people in achieving their goals. Participate in the on-going review of the Peer Support Worker’s role and contribute to the trust-wide process.
• Work flexibly as required by the individual and the Employer, which may require working out of ‘normal office hours’.
• To support people in using their individual budget (if appropriate) to maximise independence and choice. To help people overcome barriers to social inclusion by supporting them to identify and challenge mental health stigma and discrimination.
• Where relevant support or undertake carers assessments.
• To work with the person and MDT to develop their Wellness Recovery Support Action Plan
• Use own initiative, personal experience and job related training in deciding of the approach and interventions required when working with a service user in delivering peer support, although supervision is available.
• Support other members of the multi-disciplinary team in promoting a recovery orientated environment and in identifying recovery focused activities imparting information/education as required.Abide by the PSW Code of Conduct so that the central focus of your work to inspire recovery (where appropriate) is not compromised in any way.
