TITLE: Band 3 Rehabilitation Assistant – Community Neurological Conditions Management Team (CNCMT) The Royal Free Neurological Rehabilitation Centre
Location:Based within the Community Neurological Conditions Management Team (CNCMT) at the Royal Free Neurological Rehabilitation Centre (NRC), Edgware Community Hospital, Burnt Oak Broadway, Edgware, HA8 0AD (Outer London allowance).
We have an exciting opportunity for a motivated individual, who enjoys working with people to join our Community Neurological Conditions Management Team (CNCMT). The teamis highly regarded locally and within the wider neuro community, and provides a range of specialist neurological outpatient and community services primarily to Barnet residents.
You will be joining an innovative and dedicated multidisciplinary team and be responsible forsupporting the MDT in the delivery of therapy intervention following assessment. You will be seeing clients within our outpatient facilities, as well as in their homes or in the local community; supporting participation in a range of groups, carrying out treatment plans to support clients to achieve agreed goals. You will also support the team with relevant administrative support.
This position presents an excellent opportunity to further develop your clinical skills within a healthcare setting and will provide you with anexcellent opportunity to develop skills within neurological rehabilitation. The team provides a supportive environment to develop within and offers peer support from three other rehabilitation assistants. You will also be supervised and overseen by the highly skilled MDT.
We are looking for anenthusiastic individual withgood general education to GCSE level (or equivalent), as well as previous experience of working with people in a health or social care setting. Applicants must demonstrate excellent communication skills, the ability to work well in a team.
As this is a community based role, the ability to travel between sites in a timely manner is essential.

CLINICAL RESPONSIBILITIES AND DUTIES
• Provide a high standard of clinical assistance to any member of the MDT and participate actively in a variety of joint sessions with the patient.
• On instruction from a qualified member of the MDT, to carry out a range of prescribed individual sessions with patients to implement rehabilitation or condition management programmes, without direct supervision but under the instruction of a qualified staff member. Some examples include:
§ Individual upper and lower limb exercise programmes
§ Splinting/casting programmes
§ Practicing safe transfers using manual handling equipment and aids.
§ Practicing safe mobilisation using appropriate mobility aids/ orthosis.
§ Basic nursing observations
§ Maintain appropriate nursing documentation
§ Individual/group cognitive strategy training
§ Washing and dressing practice with patient
§ To assist the Occupational Therapist in home assessments
· Supporting clients motivation and engagement with rehabilitation
· To assist the Neuro psychologist to monitor mood.
· Using communication aids/strategies to achieve agreed goals
· Exercises to support patients with swallowing difficulties
· Supporting optimal nutrition to support rehabilitation
· Assisting the dietician to monitor weight and nutritional intake

· To assess and provide assistive equipment, demonstrating competency to supervisor.
· To accept responsibility for delegated departmental clinical and administrative duties.
· To support qualified staff within case co-ordinator role.
· To observe, record and verbally report the patient’s progress and contribute to goal setting and treatment revision discussions.
· To progress treatment programmes within the boundaries previously agreed with the qualified staff member.
· To demonstrate the ability to work alone, ensuring that all clinical contact carried out incorporates risk assessment and appropriate management of risk.
· To be responsible for the management, transport and fitting of specialised equipment in the hospital and patient’s community setting as required.
· To be involved in multi-disciplinary working including nursing and therapy handover, goal-setting, outcome measures and multi-disciplinary team meetings as appropriate.
· To be responsible for acquiring clinical and professional skills according to the competency framework within each setting.
· To incorporate any other duties in line with the grade and scope of the post.
