Job Summary:
To work as part of the Diabetes Service under the direction of the Diabetes Clinical Manager to support children and families, and deliver nursing care to children and their carers, within their own homes, the community and in Clinic settings.

Responsibilities:
1. In collaboration with the Diabetes team, take responsibility for the management of children with diabetes in the community or as an attendee to the Trust for inpatient or outpatient care.
2. Assist in delivering specialist diabetes advice and support to children and families affected by diabetes, maximising the child’s and carer’s knowledge to manage their diabetes.
3. Promote best practice in the treatment and care of children with diabetes.
4. Work in partnership with the multidisciplinary team, children and their families, ensuring effective and appropriate communication at all times.
5. Act as a specialist resource for all staff within the Trust, both in the hospital and within the community.
6. Help undertake research and audit to support the development of the diabetes service.
7. Contribute toward the development of strong networks with local, regional and national voluntary and statutory bodies, in order to provide individually tailored high quality care.
8. Act up in the absence of the Nurse Specialist.
