About the Role:
Our NHS Hospital client is seeking an experienced Clinical Pharmacist to join their dynamic NHS hospital pharmacy team in Hertfordshire. This role is primarily based within the Rehabilitation and Care of the Elderly wards. As a Band 7 Clinical Pharmacist, you will play a pivotal role in supporting the multidisciplinary team, ensuring the safe and effective use of medications for patients in these vital care areas.
This is an exciting opportunity to contribute to patient care in a rewarding environment, working within a well-established pharmacy team. You will provide clinical input, medication reviews, and contribute to the overall management of medicines on the wards.
Key Responsibilities:
1. Provide clinical pharmacy services on the Rehabilitation and Care of the Elderly wards.
2. Conduct medication reviews and provide advice to clinical teams on drug therapy.
3. Work closely with multidisciplinary teams to optimise patient care.
4. Ensure compliance with hospital policies and protocols regarding medication safety.
5. Deliver training and educational support to other healthcare professionals.
About You:
To be considered for this role, you should have:
1. Proven experience working on wards within the NHS (ideally in Rehabilitation or Care of the Elderly).
2. Clinical Pharmacy experience in a similar hospital setting, preferably at Band 7 level.
3. A valid registration with the GPhC (General Pharmaceutical Council).
4. Excellent communication skills with a patient-centred approach.
5. A full driving licence and access to a car, as the role involves working across two sites.
