Job overview
This post is critical to the development and delivery of effective and high standard Continuing Healthcare provision within Haringey and Islington. The post holder will hold a caseload of highly complex individuals requiring bespoke case-management, and will be responsible for clinical case management, assessments and reviews of people known to the Continuing Healthcare team. Working autonomously, the post holder will have continuing responsibility for proactively monitoring and managing individuals on the caseload of the team, and will use clinical reasoning for the effective identification of any exacerbations or to avoid unnecessary hospital admissions.
The post holder will have specialist knowledge in relation to continuing healthcare, underpinned by theory and clinical expertise, and will work in partnership with other disciplines and agencies, including GPs, social services, community nursing and therapists, pharmacists, care agencies, care homes and mental health services in order to manage the care of people with highly complex needs.
Main duties of the job
1. To work collaboratively with key stakeholders to influence and implement Continuing Healthcare characteristics and policy, reporting any gaps in provision to the CHC Operational Lead Nurse and the CCG Continuing Healthcare Lead.
2. To work collaboratively with district nurses, and offer advice and support to manage the care of individuals with complex health needs living in their own homes, or in residential/nursing care settings.
3. To ensure contemporaneous and accurate record keeping is maintained.
4. To be a key contributor to Islington's continuing healthcare panel in the absence of the CHC Operational Lead Nurse, ensuring continuity and equitable access to available resources to eligible service users and individuals.
5. To provide clinical advice to multidisciplinary teams managing the care of Islington individuals whose care is funded by continuing healthcare, including people in placements that are out of borough.
Detailed job description and main responsibilities
1. To deputise for the CHC Operational Lead Nurse in their absence.
2. To manage the junior nurse assessors in the team.
3. To organise meetings with multi-disciplinary teams (MDT) within the service and key staff members whenever required.
4. To support junior staff members with their caseloads and capacity management.
5. To work collaboratively with the CHC Operational Lead Nurse in managing the financial budget and control and adhering to good clinical practice at all times.
6. To ensure all data collection is up to date and provided in a timely manner to the CHC Operational Lead Nurse and the CCG for performance review as required.
7. To respond to complaints in accordance with the Trust guidelines and policies, ensuring any actions required are carried out.
8. To be responsible for ensuring effective multidisciplinary team working and the development of positive multi-agency working.
9. To ensure that all individual records are holistic, accurate, individualised and updated regularly.
10. To monitor all aspects of adverse events and implement changes to prevent reoccurrence and improve individual care.
11. To have the knowledge and experience to challenge traditional clinical practices to ensure that evidence based best practice is identified and implemented.
