Main duties of the job o To work as part of a team that provides mental health services, to positively promote recovery and social inclusion by focusing on the direct needs of service users, working across boundaries of care, organisations and roles o To provide support directly and flexibility to service users towards goals defined in his or her care o To create specific care plans aimed at promoting social engagement, healthy living and independence o Maintain information and knowledge of and links with community resources and actively support service users to use these resources, thereby countering discrimination and encouraging social inclusion o Planning and organisation of activities, care plans and programmes of care for Service Users under the Community Mental Health Team in collaboration with the Service User, Carer, Care Co-ordinator About us Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area.
