We are looking for an experienced Nurse to join our teams as a Band 8A Advanced Nurse Practitioner in HIV.
This is a cross site role covering Kobler HIV outpatient clinic at our Chelsea site and the HIV outpatient service at Sexual Health Hounslow based in Twickenham House at West Middlesex University Hospital.
We are looking for someone who is motivated, enthusiastic and who possesses excellent interpersonal and communication skills along with the ability to work effectively and provide leadership in the multidisciplinary team across an innovative HIV service.

The Advanced Nurse Practitioner in HIV will use their specialist knowledge, skills and experience in providing HIV care to undifferentiated patients attending the HIV departments within the HIV, Sexual Health and Dermatology Directorate. They will work as a self-directed independent practitioner managing a defined caseload of patients with complex needs for regular follow-up care and treatment, providing care which focuses on enabling patients to optimise their health within the limitations of a chronic and life-threatening disease.
The ANP will inform the strategic direction of the HIV Directorate regarding the continued development of nurse led HIV care. They will coordinate effective communication across the multi professional team, between departments and the community. The ANP will provide specialist education and development for nursing staff and colleagues through education programmes, individual development programmes and clinical supervision.
The ANP will support the Nurse Practitioner team, the nursing team and junior doctors to complete competency-based training programmes appropriate to their level. The ANMP promotes excellence in nursing practice through the integration of expert clinical practice, education, consultancy, acting as a resource, developing practice and commitment to research and audit.

Clinical
· Manage a case load of patients including those with complex treatment and psycho-social needs for on-going care
· Play a key role in the delivery and development of a high quality and innovative HIV clinical service in Kobler HIV MDT
· Assess and interpret complex clinical information and data in partnership with the patient to make appropriate diagnoses, developing and implementing a plan of healthcare management including prescribing medications or refer for medical review if required.
· Supervise junior nursing staff in treatment support, supporting them to assess and support patients starting or switching therapy and advising on those patients experiencing problems with adherence both within the outpatient and inpatient settings.
· Practice as a Non-Medical Prescriber.
· Use highly developed communication skills to assess situations and empathetically impart information, which is often complex and sensitive, at all levels of interaction with others.
· Order specific diagnostic tests and investigations, interpret findings and provide results to patients as per protocol.
· Use expertise in the emergency presentation of HIV-related problems, according to an agreed protocol, to provide a telephone triage service to assess patients’ immediate needs and direct them to the appropriate service
· Assess and support patients experiencing problems with adherence and provide treatment support.
· Advise on the promotion of health and the prevention of illness, with particular reference to smoking cessation, risk behaviours, diet, immunisation, sexually transmitted infections and HIV.
· Take a detailed sexual health history where necessary, and refer for sexual health screening, as appropriate.
· Recognise and act upon situations that may be detrimental to the health and wellbeing of the individual.
· Use highly developed knowledge and skills to assist and support the patient and their significant others in developing their understanding and ability to ensure effective self-management of HIV as a chronic disease.
· Make referrals as appropriate to members of the HIV MDT team including HIV Peer Support
· Act as a clinical leader and assist in the development of clinical skills within the wider nursingteam.
· Lead initiatives that enhance the quality of nurse-led patient care and treatment support through policy and guideline development based on the best available evidence.
· Ensure the safe custody, maintenance and administration of medication, in accordance with established Trust policy
