£32,967 - £37,152 per annum, pro rata plus 30% enhancement for nights and weekends

About the role:
This is an excellent opportunity for an experienced nurse to join our busy but very friendly community team at St Johns Hospice. If you are an enthusiastic and committed Senior registered nurse who is interested in a rewarding career within palliative care we'd like to hear from you!
The role encompasses a 24 hour telephone support line, working alongside Nurses or Health Care Assistants. You will provide palliative care support and advice to our Patients on the Palliative Care Hub register, their families and other care professionals whilst signposting to other agencies as and when required. We will require excellent assessment, planning, referral, implementation and evaluation skills to ensure our patients receive high standards of individualised care. You will also be required to support and lead some of the HCAs and be responsible for the allocation of the teams visits.
Main duties of the job
About You:
NMC registered, with post-registration experience in palliative, end of life care.
Ability to be flexible, positive and work within the wider team.
Excellent soft communication skills.
Ability to lead and empower others.

If you are an enthusiastic and committed Senior NMC registered nurse who is interested in a rewarding career within palliative care we'd like to hear from you!
This is a non-patient facing role where you will operate our 24 hour telephone support line working alongside another Nurse or Nursing Assistant.
You will provide palliative care support and advice to our Patients on the Palliative Care Hub register, their families and other care professionals whilst signposting to other agencies as and when required.
Therefore you will require excellent Assessment, planning, implementation and evaluation skills to ensure our patients receive high standards of personalised care.
You will also be required to support and lead the visiting Nursing Assistants and be responsible for the allocation of the teams visits.
Shifts are either - or 8pm - 8am (with a 30 minute unpaid break).
