If you answered yes, then come join us. We are seeking a physio, OT, or nurse to work in the Wychavon area, delivering operational and clinical leadership to the team. The Reablement team assists patients requiring additional support and rehabilitation on discharge from hospital, and those in the community who need increased care or therapy input. Assessments are holistic, person-centred, and focused on maximising independence. Patients are at the heart of everything we do.
This is a full-time post, with part-time/job share considered.
Main duties of the job
As a reablement team member, you will:
1. Work closely with healthcare and social care teams to deliver a blended approach to patient care, with the unique opportunity to experience both sectors whilst with us.
2. Perform holistic, goal-focused, patient-centred assessments, and develop thorough treatment programmes across all core specialities. This post blends clinical and operational responsibilities, giving the best of both worlds!
3. Provide operational leadership and management to a team of OTs, Physios, Nurses, and therapy assistants, supporting with complex case work and clinical development.
4. Lead on service development projects, with a focus on quality and patient flow.
5. Regular clinical supervision and professional support are provided. We positively encourage any special interests which complement the patient demographic that we see.

Job responsibilities
The post holder will:
1. Lead, co-ordinate and develop the provision of an effective and efficient integrated care service for patients primarily in the older adult population who have multiple comorbidities within the extended primary care pathway of a designated team.
2. Provide day-to-day leadership, clinical support and supervision of the Reablement team staff within the integrated team as well as supporting the integrated community service lead to provide a seamless service right across the patient pathway.
3. Support the effective triaging of patients referred to the integrated team on a daily basis, supporting the co-ordinated care delivery across the patient pathway.
4. Provide expert clinical holistic assessment and evidence-based pro-active and reactive health care interventions to support patients and their carers to achieve maximum health, wellbeing and independence.
5. Lead the integrated team in the implementation of integrated multidisciplinary care planning.

Person Specification
Experience
* Significant experience of working within community and primary care settings.
* Experience of Leadership.
* Experience of working in a multi-disciplinary team.
* Experience implementing change in clinical practice.
* Experience of active engagement in audit and clinical governance activities.
* Relevant experience at band 7.
Knowledge
* In depth working knowledge of risk assessment and evidence-based clinical skills.
* Understanding of professional and ethical boundaries and code of conduct.
* High level of knowledge underpinning roles and responsibilities of the professional groups within an integrated team.
* A high level of awareness of research processes and Evidence Based Practice.
Additional Criteria
* Able to meet the travel requirements of the role.
Qualifications
* Registered Practitioner with current registration.
* Evidence of relevant post-graduate courses / Continual Professional Development maintained in portfolio.
* I understand that I will be asked to provide original certificates to verify my qualifications, at point of offer.
* Masters Qualification or working towards.
Skills and Abilities
* Ability to communicate clearly and effectively in English in both verbal and written formats.
* Ability to make decisions autonomously, but liaise closely with other care providers within the sector.
* Ability to give clear and concise reports against performance targets.
* Excellent leadership skills.
* Ability to organise, prioritise and time manage own workload and that of the rest of the team including use of effective delegation.

* Self-motivated / directed and able to motivate others.

Salary: £46,148 to £52,809 a year pro rata for part-time.
