Fixed Term - Carers GP Support Officer - Band 4
This role will be within the Surrey Joint Carers Programme Team whose aim is to improve the lives of unpaid carers of all ages in Surrey. The Carers GP Support Officer role has been created to support GP practices and Primary Care Networks embed best practice, increase visibility of carers and young carers and raise awareness. The officers will build professional relationships with a wide range of stakeholders including Social Care Locality Teams, Social Prescribing Teams, Carer Support service providers and national carer bodies.
Main duties of the job
These will include:
* Raise the profile of unpaid carers across Surrey being a key contact for GP practices and Primary Care Network Managers
* Support and promote the GP Carer Personal Health Budget programme
* Support Primary Care to identify carers of all ages, ensuring that staff know the support offer for unpaid carers in Surrey
* Encourage and support GP practices to obtain the RCGP Veteran Friendly accreditation
* Deliver carer awareness training to staff within practices and Primary Care Networks
* Attend and assist with chairing responsibilities in the Staff Unpaid Carers Network
* Promotion and support the participation in the GP Carer Quality Marker returns

Person Specification
Qualifications and training
* Educated to NVQ4/Diploma or equivalent experience of working in a relevant area

* Experience in a health and social care environment
* Experience in organising and delivering training
* Track record of engaging and working with a range of stakeholders, including health and social care providers, and community specialist groups

* Knowledge of key issues affecting carers
* Good presentation skills to a range of audiences
* Able to handle sensitive and confidential data and to recognise the need for confidentiality
* Able to identify the need to escalate concerns to appropriate personnel
* Effective use of Microsoft Office products

£26,530 to £29,114 a year. Per annum, pro rata. Including 5% High Cost Area Allowance.
