Either 2 part-time posts x 18 hours per week or 1 full-time post at 36 hours per week.
The Rapid Response service is looking for Rapid Response Assistants to work in a multi-disciplinary team of professionals delivering a 24-hour response service to avoid hospital admission and facilitate hospital discharges. You would carry out person-centered care offering practical support and advice.
This is a developing service with great career prospects and training on offer; you must hold a valid driving license and have access to a car.

The service is open Monday to Sunday from 8 am to 10 pm, 365 days a year, including Bank Holidays.
