Community Specialist Physiotherapist or Occupational Therapist - Crewe

This post is within the Crewe Care Community and can be either an Occupational Therapist or Physiotherapist. The role is to assess, manage and provide treatment to a caseload of service users with complex and/or multiple needs. They will be working closely with the MDT and provide supervision to therapy assistants, junior staff and students. There may be an expectation to work between multiple sites in the organisation.

The physiotherapist or Occupational therapist will be expected to join the 7-day, urgent community response rota and may be lone working within the guidelines of the lone working policy.
This is a static permanent post.
Main duties of the job
We are looking for enthusiastic, motivated and flexible physiotherapist or Occupational Therapist to join our friendly, supportive, dynamic team in Crewe Care Community/ CCICP.
We provide a rapid, unplanned and planned response, we facilitate early discharge from hospital and aim to reduce hospital admissions. We also cover discharge to assess beds.
There is a varied caseload and you will need to demonstrate knowledge, experience and relevant clinical skills with patients with complex / long term conditions, falls, orthopaedics, general mobility problems and neurological conditions.
The successful candidate will work as a member of our integrated multidisciplinary team alongside District Nurses, Advance Nurse Practitioners, Complex Case Practitioners, MacMillan Nurses and will be responsible for supervising junior members of staff and students.
There will be associated administrative duties and you will need to be computer literate with a good knowledge of Microsoft packages and have accurate keyboard skills. Our team utilises digital technology to support remote working and accommodate a flexible working approach where possible and is supported to do this with access to a laptop and mobile phone.
You are required to be a car driver and have access to your own vehicle.

Job responsibilities
Clinical Practice
1. Manage a clinical caseload with autonomy, work without direct supervision.
2. Undertake comprehensive physiotherapy assessment, provide a therapeutic diagnosis, and implement agreed treatment plans.
3. Provide individual or group therapy as appropriate to setting.
4. Implement monitoring and evaluation of treatment to measure progress and outcomes.
5. Develop programmes of care to support discharge from service.
6. Be responsible for equipment used in carrying out clinical duties.
7. Record, maintain and store accurate documentation.
8. Arrange appointments and work closely with MDT.
9. Contribute to the safeguarding of our Service users/families.
10. Assess capacity, gain valid informed consent, and work within a legal framework.
11. Maintain professional registration with HCPC.
Leadership and Management
1. Communicate effectively with a range of professionals.
2. Assist the Managers in achieving departmental objectives.
3. Participate in professional development programmes.
Education
1. Organise and lead in-service training programmes.
2. Critically evaluate current research and apply to practice.
3. Train, supervise and support less experienced therapy staff.
Research, Audit and Governance
1. Undertake measurement and evaluation of work through evidence-based practice.
2. Support research activity with the service.

Qualifications
* Degree in physiotherapy
Registration
* HCPC registered
* CSP registered
Experience
* Previous experience of working in the NHS
* Experience of working in a community setting
* Completion of band 5 rotation
* Student educator or experience of having students
* Experience of audits
Communication
* Good written and verbal communication skills
