Job Summary
CPG Rehab, Re-enablement and Recovery unit is a bed-based facility supporting people to recover and regain their independence following a period of illness, trauma, surgery, or significant event. It supports hospital discharges, prevents admission to hospital or long-term care and promotes independent living. It is a multi-disciplinary service with Nurses, Physiotherapists, Occupational Therapists, Social Workers, and Care Staff.
Title: Social Worker
Base: Community Inpatient Unit
Hours: 37.5 a week, permanent
Salary Range: £29,734 - £34,474 per annum.
As a Social Worker for the Rehab and Recovery unit, you will work as part of the wider MDT in the admission, care, and discharge of a range of patients. You will manage and co-ordinate a defined caseload of adults and their carers who are within the unit. Duties will include identifying and assessing need and developing support plans, assessing and managing risk, which may include cases where admission to long-term care is necessary. Day to day, you will communicate and negotiate with individuals, carers, and external providers on the level and content of care packages.
You will have a key role in promoting independence, with a strong focus on community resources and outcomes while ensuring the protection of vulnerable people from harm or abuse.

Person Specification
Experience Essential:
* Experience of working with adults.
* Working knowledge of relevant legislation.
* Assessing people, the development of support plans, and reviewing such plans.
* Work in a multi-disciplinary team.
Desirable:
* Experience of using SystmOne.
* Assess complex situations of risk and admission to care.
Qualifications Essential:
* Relevant social work qualification.
* Relevant registration with a professional body.
