Would you like to be a crucial part of an innovative project? An exciting opportunity has arisen for Health Care Support Workers to join the Shropshire, Telford & Wrekin Virtual Wards across all the localities.
The Virtual Ward is a system-wide initiative, working in partnership to provide our patients with access to rapid assessment, treatment, virtual monitoring, and care planning to stabilise and support an acute episode of ill health or a deteriorating chronic condition that would alternatively require admission to hospital.
The service offers an appropriate alternative to admission pathway, which aims to deliver a recommended and agreed medical, nursing & therapeutic treatment plan. The focus is supporting the safe management of people in their own home. The role and service offer integration and partnership working across the system to ensure a seamless service.
Full-time and part-time positions are available.

You will support individuals and clinical teams to deliver high-quality, clinically effective patient care through assessment, planning, implementation, and evaluation. Along the way, you'll build strong relationships with a wide range of healthcare professionals and patients.
