Manage a clinical caseload and provide support and oversee junior staff who are associated with a caseload Provide direction and leadership to junior staff including but not limited to management supervision and clinical oversight. Assist junior staff to prioritise, implement and evaluate patient care in accordance with Trust policies, procedure protocols and guidelines, while adhering to these yourself. To complete a holistic assessment of the CYP identifying their nursing needs. To develop implement and evaluate appropriate care plans and comprehensive packages of care. To provide quality care to children in partnership with families and carers as commissioned and described within the service specifications. To deliver direct patient care to meet the physical and emotional needs of the child / young person, taking account of specialised (enteral) feeding, respiratory (tracheostomy, ventilator), manual handling and medication regimes (sub-cutaneous infusions, Intravenous). To take all possible steps to ensure the safety and welfare of children and staff To support the service with managing emergency admissions, discharges and reallocation of staff and patients To carry out all relevant aspects of care adhering to NCH&C Trust Policy/procedure and Nursing and Midwifery Council guidelines To undertake risk assessments and complete Datix reporting as required. To communicate effectively with child, parents/ carers and the wider multi-agency staff regarding the childs wellbeing needs to ensure continuity of care. To offer advice, guidance and reassurance to families as needed To attend, contribute and chair team meetings, attend Trust meetings, training sessions and courses as required. To act as a specialist support, educator and resource to all other relevant professionals To provide, implement and regularly assess the teaching of specific nursing techniques, including invasive procedures to identified staff/professionals/parents/carers/student nurses/apprentices in a variety of settings. To work proactively to contribute to the EHCP process To actively engage in the development of protocols and guidelines and care pathways for the management of children with complex and/or acute health needs. Promote and develop the use of current evidence to inform the delivery; and promote the development of clinically effective and safe care. Provide initial triage and assessment for new referrals to the service, delegating to appropriate others as required. Lead and develop effective discharge from hospital to home or respite setting, carrying out risk assessments in line with continuous care assessment to ensure seamless transition to home or into a respite setting. Plan and allocate work in order of priority and delegate to colleagues and direct reports, to skills and competencies.
