Job Description:
An exciting opportunity has arisen for a Clinical Pharmacist to join a well-established GP practice in Salisbury. The practice is seeking a highly motivated and skilled individual who is passionate about patient care and improving the quality of healthcare delivery. As a Clinical Pharmacist, the successful candidate will work closely with the practice's clinical team to provide expert medication management, improve patient outcomes, and support the delivery of high-quality healthcare services.
Key Responsibilities:
Review patient medication lists, identify and resolve medication-related problems, and ensure optimal medication therapy management.
Provide expert clinical advice to GPs and other healthcare professionals on medication-related matters.
Conduct medication reviews and offer personalized advice to patients on their medication regimen.
Support the safe and effective prescribing of medicines, ensuring compliance with relevant guidelines and legislation.
Deliver targeted interventions, such as supporting patients with long-term conditions and providing tailored medication management plans.
Lead or participate in the development of clinical protocols related to medication management.
Contribute to the training and development of the practice team, sharing knowledge and best practices.
Work closely with local pharmacies and other healthcare providers to ensure seamless care for patients.
Key Requirements:
Registered Pharmacist with the General Pharmaceutical Council (GPhC).
Experience in a clinical or GP practice setting is desirable, but not essential.
Strong knowledge of clinical guidelines, medications, and their indications.
Excellent communication skills, with the ability to work collaboratively with multidisciplinary teams.
A patient-centered approach and a commitment to improving patient care.
Strong organizational and time-management skills, with the ability to manage a diverse and varied workload.
Desirable:
Postgraduate qualifications in Clinical Pharmacy or similar.
Experience in primary care or GP practice settings.
Knowledge of NHS systems, including prescribing practices and medication reviews.
