We are searching for an enthusiastic person to help us support frail vulnerable adults with their individual Reablement journeys to enable them to regain their independence. Key requirements for a Needs Assessment Officer: You will need to be genuinely concerned about people's quality of life and be enthusiastic and able to think creatively. You will be enabling individuals to regain their independence. You would be working with the customer, setting and achieving goals in their own homes in the community. You may be responding to emergency situations in the community in partnership with health services and Seniors in the Team. You would be working with customers supporting their personal care and administering medication and nutritional needs. Regularly review customers' progress, monitoring the effectiveness and efficiency of the support plan and supporting Seniors within the office to make adjustments as necessary. Maintaining case records, including utilising department electronic record systems. Supporting a Supervisor to run the shift. Meet and greeting customers in their own homes.
