As such, we are looking for a Social Worker who is able to take up a role in supporting our service users in delivering high quality, person centred, strength based social care duties, through empowering assessments and support for individuals within our Specialist CMHTs in North Manchester.

Main duties of the job
You will be responsible for carrying out social work duties in a fast paced multi-disciplinary team requiring the highest levels of skills, knowledge and professional expertise. You will ensure the delivery of effective, safe integrated services for adults through the promotion of high standards of practice in assessments and person-centred planning, being creative to meet individual outcomes. The role involves:
1. Assessing, planning and review in line with requirements of the Care Act (2014) and s117 of the Mental Health Act (1983).
2. Taking a strengths based and assessed based approach to support.
3. Working with a caseload of individuals in a collaborative way to support in the development of their assessments, recovery plans and structured care plans.
4. Signposting to non-statutory services or commissioning packages of care.
5. Completing high quality assessments of Mental Capacity (in line with Mental Capacity Act 2005) and chairing Best Interest Meetings.
6. Making safeguarding enquiries in line with s42 of the Care Act (2014) ensuring that safeguarding is person centred, outcomes achieved and risk mitigated.
7. Complying with SWE Professional Standards and being registered with Social Work England.
8. Complying with legislation, national and local policies and procedures.
