What will your responsibilities be?
In this role, you will undertake needs-led assessments and provide support to adults and their carers. You will offer specialist support to service users, carers, and colleagues based on your skills and qualifications. Responsibilities include completing complex multi-disciplinary assessments and providing ongoing case management for individuals with unstable and unpredictable conditions, under the supervision of Team Managers, Assistant Team Managers, or Senior Care Managers, within the criteria of each individual service.

Social Work England registration
Eligible to work in the UK
Hold a full UK licence
Post-qualified experience
