Stevenage Healthcare/Stevenage South PCN are looking for a care coordinator to join their Enhanced Health in Care Homes team on a full-time basis for 37.5 hours per week. Care coordinators play an important role within a PCN to proactively identify and work with people, including the frail/elderly and those living in care homes, to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed. Duties will include; Coordination and delivery of MDT (Multi-Disciplinary Team) meetings within the PCN Care Homes and PCN practices. Participate in the MDT meeting, collaborating with primary care, community healthcare, social care services, and other relevant professionals. Maintenance of records of agreements, and actions made in the MDT. Collaborate with the MDT to conduct personalised assessments and develop individualised care and support plans. Support individuals in managing their own care, health, and well-being. Provide coordination and navigation for people and their carers across health and social care services, working closely with other Care Coordinators, social prescribing link workers, health & wellbeing coaches, and other primary care professionals. About the Candidate The ideal applicant will have the following skills, experience and qualifications: Essential: Experience of working in health, social care and other support roles in direct contact with people, families or carers (in a paid or voluntary capacity). Experience of working within multi- professional team environments. Experience of supporting people, their families and carers in a related role. Understanding of the wider determinants of health, including social, economic and environmental factors and their impact on communities, individuals, their families and carers. Understanding of the needs of older people / adults with disabilities / long term conditions particularly in relation to promoting their independence. Basic knowledge of long term conditions and the complexities involved: medical, physical, emotional and social. High level of written and verbal communication skills. Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines. Desirable: NVQ Level 3 in adult care - advanced level or equivalent qualifications or working towards. Experience of working directly in a care coordinator role, adult health and social care, learning support or public health / health improvement. Experience or training in personalised care and support planning. Knowledge of Safeguarding Children and Vulnerable Adults policies and processes. About Us Stevenage South PCN (SSPCN) is made up of local practices who are commissioned to work together to provide a range of healthcare services to a population of 60,000 patients across Stevenage. The core network practices include Bedwell Medical Centre, King George and Manor House Surgery, Knebworth & Marymead Medical Practice and Shephall Health Centre, however other practices may join the PCN in the future. The PCN is governed by a PCN board, with representatives from each practice, who provide oversight of the service and approval for strategy and future PCN plans and objectives.
