Medical Service Line 2 is looking for a leader to work collaboratively with the well-established senior team in the Cardiology Specialist Nursing Team.
You should have substantial experience with nursing Cardiology patients and will be educated to degree level.
You will need to possess excellent communication, interpersonal skills. Ward 8 has always been at the vanguard of any new digital implementation project and any Senior Cardiology Specialist Nurse within the team must be competent or have a willingness to learn the trusts key digital systems (HealthRoster, SafeCare live and NerveCentre).

Main duties of the job
1. To provide the highest standard of holistic safe and effective care through the effective use of evidence based practice in line with agreed local and national standards.
2. Accountable for the planning and delivery of all aspects of patient care throughout the working period as set out in the NMC Code.
3. To provide a continuously improving service in line with the clinical governance framework, Trust objectives, and the NHS performance framework.
4. To provide clinical leadership, expertise, advice and support to the nursing and multi-professional team to facilitate the effective delivery of care to patients at all times.
5. To support and continually develop staff to ensure they have the appropriate level of skill, knowledge and competence to meet service/specialty requirements.

Job responsibilities
1. To provide the highest standard of holistic care for patients through the utilisation of enhanced clinical skills and advice in the assessment and management of patients in collaboration with the multi-disciplinary team to improve and maintain the quality and safety of patient care.
2. To contribute to the delivery and development of Cardiology Service through the use of effective evidence based practice in line with agreed national standards for clinical practice.
3. To be accountable for the planning and delivery of all aspects of patient care in relation to the Cardiology Services and as set out in the NMC Code.
4. To continuously improve clinical care standards in line with the clinical governance framework to ensure the highest quality of care is provided in the safest manner.
5. To work as a senior member of a clinical team, in the provision of Cardiology Services.
6. To provide specialist clinical advice and support to clinical staff within GHNT on the management and care of patients with Cardiology complaints to facilitate the effective delivery of care to patients.
7. To communicate effectively with all members of the multi-disciplinary team, patients, carers and the public.
8. To act as a resource and role model for the profession at all times.
9. To provide education and training to clinical staff and internal/external agencies.
10. To manage own case load of patients and independently review patients in clinic settings with various cardiology conditions.
11. Support the provision of Heart Failure knowledge and advice to other nursing staff, the wider multi-professional team, allied health professionals, patients and their families/carers.
12. Support the provision of education and training for nursing and allied health professionals; including health promotion.
13. The post holder will be a senior member of the team who provides high quality specialist Cardiology Care.
14. Communicate with medical staff and other health professionals regarding patient conditions and clinical care plans, disseminating this information to the wider clinical team.
15. Ensure effective communication with patients, relatives and carers, explaining medical conditions, progress and investigations taking into consideration level of understanding, culture and background.
16. Encourage and support patients to take an active part in their own care.
17. Communicate with staff at all levels to motivate and advise on achieving high standards of care.
18. Maintain effective links with specialist nurses, utilising their skills and knowledge.
19. Regularly give presentations to groups for education and training.
20. Represent the specialism/clinical area at appropriate forums, cascading information both up and down the organisation.
21. Support relatives to take part in the care and welfare of the patient.
22. Demonstrate a caring, nurturing attitude at all times with patients, carers and relatives.
23. Actively participate in professional forums including ward/departmental meetings and multi-professional meetings.
Minimum Requirements
* Current RN NMC Registration.
* Degree in Nursing.
* Relevant experience within Heart Failure Management.
* High level of interpersonal and communication skills.
* Non-Medical prescribing.
* Organisational skills.
* Evidence of ongoing Professional Development and Clinical Practice Development (CPD).
* Significant Band 6 experience within Heart Failure Management.
* Evidence of CPD in specialist area.
* Ability to facilitate learning to others - teaching qualification.
* Ability to cope with and diffuse difficult and stressful situations.
* Evidence of an ability to prioritise their own workload.
* Risk management knowledge.
* Flexible approach to work.
* Participation in Clinical Supervision.
* To work across primary and secondary care settings.
* Recognised National Qualification In Speciality.
* Counselling skills.
* Audit/research experience.
* Experience of change management.
* Experience of managing own patient case load independently.
