We are pleased to be able to offer a post as a band 6 Senior Mental Health Practitioner with the Teignbridge OPMH team.
Teignbridge OPMH provides community mental health services to people over the age of 65 with functional mental health problems and people of any age with an organic mental health problem.
Teignbridge OPMH is an established team that works to a multi-disciplinary and multi-agency model.
Main duties of the job
The post holder will deliver high quality comprehensive mental health services. The role involves managing a defined caseload with a focus on providing expert assessment and treatment for people with complex mental health difficulties and their carers. This will include key working, design and co-ordination of treatment packages that are personalised and evaluated. The liaison with a wide range of statutory and non-statutory agencies is central to this role.
The post holder will contribute to the clinical leadership of the team, providing supervision for other members and participating in team and service development, and evaluation.

Job responsibilities
* Contribute to the teams engagement with a wide range of stakeholders including staff, service users, their carers and other statutory and voluntary sector agencies.
* Impart highly complex and sensitive information to patients and families/carers regarding assessment, care planning, treatment, and review.
* Liaise with multi-disciplinary teams to ensure the provision of consistent well-coordinated care.
* Communicate with primary care teams regarding client care.
* Engage with external agencies involved in the provision of care, e.g., voluntary and private sector, police, social services, housing, employment, etc.

* Deliver care in line with evidence-based practice and employ engagement and therapeutic skills appropriate to the clinical situation presented.
* Responsible for triaging individuals and signposting to the most appropriate services.
* Provide specialist advice to referrers/other multi-disciplinary staff regarding the care of individuals.
* Enable communication and engagement of individuals and their carers to ensure they have a voice about the services they receive and how these are developed.
* Ensure appropriate action is taken to safeguard adults and children, including providing and receiving highly complex and sensitive information.
* Ensure assessment and treatment plans are holistic, outcome-focused, and take into account the physical health and social needs of the individual.
* Monitor and review mental health medication considering interactions with physical health needs.
* Maintain knowledge of eligibility for services including FAB/FACS assessments and refer appropriately.
* Ensure timely and accurate documentation of all clinical activity in line with professional guidance and best practice.
* Practice in a way that actively minimizes dependency and promotes recovery.
* Use specialist knowledge (e.g., Non-Medical Prescribing, CBT) in day-to-day practice.
* Participate in audit and research programs within the team as agreed by the Directorate and implement findings.
* Involve in developing local systems and processes ensuring implementation of Trust policies.
* Participate in the team's compliance with CQC standards through the development of provider compliance assessment tools.
* Contribute to clinical governance arrangements ensuring quality of service provided.

Person Specification
Qualifications
* A professional mental health qualification and registration (e.g., RMN, BSc/Diploma in OT, SROT, Social Work Degree, or equivalent, Community Nursing Degree).
* Non-Medical Prescriber and/or another recognised therapy qualification.
* Postgraduate qualification relevant to specialism.
* Training to level 3 in Safeguarding Adults and Children and Young People: roles and competencies for Health Care staff.
Experience
* Substantial experience of managing an extensive and complex caseload of clients with a range of complex mental health presentations.
* Experience working within a community mental health setting in either the statutory or voluntary sector.
* Providing clinical supervision.
* Experience of working in a multi-disciplinary and multi-agency environment.
* Participation in user involvement in services.
* Experience of delivering model-based psychological therapy.
