Base: EAST Integrated Community Network Team, Bridgend
An excellent opportunity is offered for an experienced Community Nurse to apply for the post of Band 6 District Nurse Caseload Holder.
The successful applicant will hold responsibility for the care and management of a defined clinical caseload of patients within a very busy District Nursing Team.
The post holder will find themselves well supported by an inter-disciplinary team, which includes social work, occupational therapy, physiotherapy, speech and language therapy, dietetics, and pharmacy.
Main duties of the job
The post holder will be responsible for providing support to the Team Leader in the day-to-day management of multidisciplinary/multi-agency packages of scheduled and unscheduled care for patients with multiple, complex, and palliative care needs.
Assisting the Team Leader in the requirements of the nursing team by utilizing effective management and leadership skills.
Responsible for the delegation of clinical nursing activities to appropriately skilled team members, ensuring that an efficient and effective service is delivered to the practice population.
Responsible for prioritizing delegated home visits according to the patients’ clinical need and geographical location.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications / Knowledge
* Has or is working towards DN SPQ
* Able to effectively use the Civica scheduling tool
* Illustrates a sound knowledge of effective caseload management
* Understand role in supporting Band 7
* Is committed to own and team CPD
* Working knowledge of safeguarding procedures
* Understanding of the DN interim principles and how these relate to practice
* Full range of pertinent clinical skills
* Ability to recognize and act accordingly when risk is identified.
* Understand the role of the MDT and make pertinent referrals.
