Job summary We are recruiting for a full time Band 6 CPN to join our team, working within the Denbighshire Older Persons Community Mental Health Team. Main duties of the job As Care Coordinator, you will hold a caseload of patients with organic and functional mental health needs, travelling to see them in their own homes, in care homes and in-patient settings. If you are an experienced Mental Health Nurse with passion and enthusiasm for working with older adults, then we would love to hear from you.
