Advanced Clinical Practitioner Clinical Educator
Building on Haxby Groups experience as a regional workforce and training hub alongside a vision to become an at-scale multiprofessional primary care training provider, this innovative role will allow qualified Advanced Clinical Practitioners with an interest in education to maintain and develop existing clinical skills in a supportive practice environment alongside an expanding educational portfolio. A proportion of the successful candidate's week will be spent working as part of the Haxby Group Training team. This will involve the development and delivery of teaching and leadership skills, helping to mentor and support both current and aspiring practice nurse and other health professionals.
Main duties of the job
In the clinical part of the role you will be responsible for delivering quality holistic care and treatment to the Practice population. Whilst you are a dependent practitioner, you are able to work independently and make independent decisions enabled by a collaborative and supportive working relationship with your clinical supervisor. You will demonstrate safe and effective clinical decision making and expert care, including assessment, diagnostic and management skills. As well as dealing with acute illness, you may be involved in the management of long-term conditions.
You will be responsible for supporting, designing and delivering training programs that will meet the needs of our customers. You will perform training needs assessments, design and deliver curricula and manage all phases of training interventions. You will support the development of education and training opportunities, projects and schemes within the remit of Haxby Group Training and the Primary Care Workforce and Training Hub (PCWTH), liaising with practices across Humber and North Yorkshire to provide support and guidance relating to students and training. You will supervise other clinicians within our local and regional training network.

As a Primary Care Training and Workforce Hub in Humber Coast and Vale, we also provide support for a growing network of local practices, co-ordinating local schemes on behalf of Health Education England.

Job responsibilities
Clinical Responsibilities:
* Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis within your sphere of competence
* Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
* Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs
* Utilise your clinical supervisor or mentor and other clinical staff appropriately to ensure safe and effective patient care, demonstrating awareness of your own limitations
* Provide safe, evidence-based, cost-effective, individualised patient care within the surgery, patients own home or other environment where patient care is carried out.
* Refer patients directly to other services or agencies as appropriate, utilising Practice and local guidelines and in a timely manner
* Pro-actively identify, diagnose, monitor and manage treatment plans for patients at risk of developing a long-term condition as appropriate
* Diagnose and manage acute conditions, integrating both drug- and non-drug based treatment methods into a management plan
* Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, and national and practice protocols, and within own scope of practice.
* Work with patients in order to support compliance with and adherence to prescribed treatments.
* Provide information and advice on prescribed and over-the-counter medication on medication regimens, side-effects and interactions

Teaching and Training Responsibilities:
* Pro-actively working, leading when required, to support and develop the Primary Care Workforce and Training Hub (PCWTH) hosted by Haxby Group.
* Support in the development of education and training opportunities, projects and schemes within the remit of Haxby Group Training and the PCWTH
* Supporting with Practice Learning Facilitator work, liaising with practices and education providers across Humber and North Yorkshire to provide support and guidance relating to students and training.
* Identifying and assessing future and current training opportunities and needs.
* Translating requirements into training services and products that meet opportunities and customer needs.
* Maintaining a keen understanding of training trends, developments and best practice.
* Developing a training services plan and assist in building the annual training delivery programme.
* Train the trainers role: directly support and aid development of other trainers
* Preparing or overseeing teaching plans.
* Delivering training and courses, deploying a wide variety of training methods delivering content in keeping with relevant local and national evidence and guidance.
* Monitoring and evaluating training programmes effectiveness, success and return on investment and reporting on them.
* Identifying and assessing future and current training needs through job analysis, career paths, annual performance appraisals and consultation with line managers.
* Assist with or lead on projects as appropriate to develop new business opportunities regionally, nationally and internationally.
* Fostering the development of inter-professional learning and the integration of students and trainees into the workplace.
* Liaising with other members of the Haxby Group Training team and key Haxby Group staff to ensure high quality educational provision.
* Being aware of key performance requirements and targets regarding PCWTH and other training commitments and assist in ensuring they are met in a timely manner.
* Developing and maintaining key links between Haxby Group Training and other stakeholders such as educational establishments or organisations, NHS organisational bodies, non-NHS establishments, other practices, actual and potential business clients, and other providers.

* Registered Nurse (NMC) or Registered Paramedic (HCPC) or Registered Physiotherapist (HCPC).
* Masters Degree in Advanced Clinical Practice.
* Minor illness qualification at degree level assessed by OSCEs.
* Current registration.
* Independent non-medical prescriber or working towards it or proven ability to use Patient Group Directions.
* Aware of accountability of own role and that required of a Registered Practitioner.

* At least two years experience post MSc qualification.
