Mid Norfolk Primary Care Network (PCN) are seeking an experienced care coordinator to join our team, working across all our sites and linked care homes, on a full-time permanent basis. As part of the PCN team, the successful candidate will play an important role in proactively identifying and working with patients with long-term conditions, to provide coordination and navigation of care and support across health and care services. Responsibilities will include: Work collaboratively with other teams and services to maintain an effective and efficient service. Plan and organise work using own initiative, whilst being able to work as a valuable team member. Make referrals to relevant service providers. Coordinate and where required chair MDT meetings, having proactively prepared any actions prior to the meeting and ensure all relevant clinical and non-clinical staff are present. Promote and facilitate integration within the GP and community teams. Provide and receive sensitive information about difficult or complex matters always respecting confidentiality. Create detailed and clear minutes from the meetings and forward to all relevant parties with actions highlighted to individuals. Offer appropriate support and guidance to patients and families/carers. Receive, record, and collate information and maintain accurate electronic records of patient care and planned service provision, including hospital admission and discharge information. Signpost service users and carers to relevant services. Contribute to quality audits. Contribute to the development of the role and systems, to effectively enhance the service provided and improve outcomes. Assist patients to be better prepared to have conversations on shared decision making, improve awareness of related support tools. Provide patients with high quality, easy to understand information to assist them in making choices about their care. Support in the delivery of enhanced services and other service requirements on behalf of the PCN. Support delivery of QOF or cost effectiveness initiatives. About the Candidate The successful applicant will have the following experience and skills; Essential: Excellent working knowledge of Microsoft office packages. Experience of dealing with sensitive/confidential information. Proven administrative experience. Understanding of clinical governance. Highly motivated. Desirable: A-level NVQ 3 or equivalent experience in admin/customer service environment. Experience of working in a healthcare setting. Experience of office procedures within an administration role. Working knowledge of the management of databases. Experience of working within multidisciplinary teams. Knowledge and understanding of relevant health and social care legislation and initiatives.
