A vacancy has arisen for a Family Partnership Practice Lead (FPPL) working within the Tonbridge and Malling district. The FPPL has a key role in embedding the Family Partnership Model within Kent Health Visiting Services under the guidance of the Family Partnership Programme Manager.

The evidence-based model supports, empowers and enables parents and families to overcome their difficulties, build strengths and resilience, and fulfill their goals more effectively. Working as part of the local Health Visiting team, the Family Partnership Practice Lead will facilitate the early identification of vulnerable families and support the delivery of the KCHFT Family Partnership Programme by managing a small caseload of families, supporting them ante-natally and until their infant is a year old. In order to facilitate the delivery of the Family Partnership Model, the post holder will have a lead role in embedding the Family Partnership Model within district health visiting practice as well as providing leadership and some line management to the district health visiting team. The work with clients is intensive and challenging but provides a real opportunity to make a difference to their lives. You need to be a resilient and flexible Health Visitor with excellent communication and team leadership skills.
Main duties of the job
The Family Partnership Practice Lead will have a key role in embedding the Family Partnership Model within Kent Health Visiting Services under the guidance of the Family Partnership Programme Manager. Working as part of the local Health Visiting team, the Family Partnership Practice Lead will facilitate the early identification of vulnerable families and support the delivery of the Family Partnership model by managing a small caseload of families. The post holder will also be a clinical leader and provide teaching, mentoring, and supervision to junior staff to ensure the effective delivery of the programme. Working collaboratively with the District Manager and Senior Health Visiting Practitioner / Practice Teacher, the Family Partnership Practice Lead will also support the delivery of the Healthy Child Programme in accordance with national and local strategy and guidance. The Family Partnership Practice Lead will work in partnership with partner agencies in the identification and protection of vulnerable children and contribute to the development of care plans, working in accordance with local and professional guidelines. They will ensure completion of agreed outcomes for delegated Children Protection cases as per agreed multi-agency care plans ensuring regular communication with the caseload holder. The Family Partnership Practice Lead will be expected to lead on case discussions in partnership with the District Manager and Senior Health Visiting Practitioner.

Person Specification
Qualifications
* Nursing or midwifery qualification and registered with the Nursing and Midwifery Council.
* Specialist Community Public Health Nurse qualification.
* Degree level or equivalent professional qualification.
* Willingness to undertake specialist training to enable the delivery of the Family Partnership model.
* Mentorship qualification.
* Educated to Masters level.
* Teaching qualification.
* Clinical supervisor.
Experience
* A minimum of 5 years' experience within the NHS or statutory sector organisation.
* Significant experience of working with vulnerable families.
* Significant experience of midwifery, child health or public health.
* Experience of working with children and families using the Healthy Child Programme.
* Able to demonstrate delivering interventions that impact individuals, families and their communities.
* Experience in managing highly complex, sensitive information to include analysis of data.
* Experience of delivering training/education.
* Good experience of multi-agency partnership working.
* Experience of conducting formal project evaluation and/or research.
* Experience of undertaking root cause analysis.
* Training and experience of conducting investigations.
Knowledge
* Knowledge and application of theories of behaviour change and motivational interviewing.
* National and local safeguarding policies and procedures.
