Main duties of the job Band 4 Assistant Practitioners Within The Complex Care at Home Team You will have already acquired or currently be working towards a minimum of a level 4 qualification in Health and/or Social Care and have extensive experience as a Band 3 HCSW. As an Assistant Practitioner you will work with a wide MDT under the delegation of a team of Registered Nurses. You will utilise and develop a wide variety of skills including airway management, care of tracheostomy & ventilator and be able to meet the emotional, physiological, and clinical needs of individuals. The role is challenging but rewarding and you will be fully encouraged and supported with your professional development. The Care at Home Team has a dynamic internal education team with a focus on professional development. The ability to drive patient vehicles and the ability to travel between sites in a timely manner is essential.
