An exciting opportunity has become available to work within the MDT environment of the Urgent Care Team supporting early hospital discharges and responding to urgent community response referrals to help prevent admissions to hospital.
You will work within the Torbay urgent care team with support and supervision from the multi-disciplinary team, providing an urgent care service to patients to promote faster recovery from illness, prevent unnecessary acute hospital admissions, support timely discharge, and maximise independent living.
You will be working towards ensuring the national targets for urgent care response are met by providing timely interventions in line with the 2-hour and 24-hour response times. This post also covers some intermediate care work to help with patient flow.
Main duties of the job
Completing assessments within patients' homes, providing equipment, and carrying out observations under the supervision of the occupational therapists, physiotherapists, and nurses.
The focus will be around hospital avoidance rapid assessment to prevent hospital admission or facilitate early discharge. We provide a 7-day service covering the hours of 8 am to 7 pm including weekends.

Qualifications and Training
* Foundation Degree (FdSc in Healthcare Practice (Nursing Associate))
* Trust induction training
* Experience in a Community Setting
Knowledge and Experience
* Understand and demonstrate evidence-based practice
* Understanding of when to seek advice and refer to a registered care
* Understanding of the scope of the role of the Nursing Associate in the context of the team and the organisation, and how the role may contribute to service development
* Understand and demonstrate the importance of the promotion of health and wellbeing
Specific Skills and Knowledge
* Ability to work independently with a delegated caseload, use initiative whilst understanding limits of scope
