Job summary We have an exciting opportunity for an experienced Advanced Nurse Practitioner or Advanced Clinical Practitioner to join our team on a part time basis working 24 -28 hours per week. The practitioner will use face to face, telephone and electronic consultations to make assessment and management decisions regarding patient care which will usually be in the practice premises but may involve home visits. The practitioner will be confident in giving advice, prescribing, using basic practical skills and referring to primary and secondary care colleagues. The practitioner will work collaboratively with the whole practice team to serve the needs of the practice population and thus good interpersonal skills are paramount. Main duties of the job The Advanced Nurse /Clinical Practitioner will be responsible for the assessment of health and clinical needs of the patient. You will be required to work autonomously in delivering independent assessment, clinical examination, diagnosis, clinical decision making and planning of care, treatment and prescribing across all age groups including young children.
