Job summary Berkeley Vale Primary Care Network (PCN) is seeking a passionate and experienced Frailty and Dementia Lead Nurse to join our multidisciplinary team. Main duties of the job Act as a senior nurse for dementia and frailty within the PCN, providing specialist assessments, advice, and management plans. Lead the development of multidisciplinary frailty and dementia services across the PCN. Support the proactive identification and management of patients with frailty. Work in collaboration with the PCN and other stakeholders to develop and improve frailty services. Provide training and education to staff on dementia and frailty to ensure they maintain up-to-date knowledge and competencies. Work in partnership with GPs, social care, and voluntary sector providers to support seamless, holistic care.
