Wellbeing Project Delivery Officer (Community Connect)
Pioneering Care Partnership (PCP) are recruiting for a Wellbeing Project Delivery Officer (Community Connect).
Responsible to: Health and Wellbeing Project Coordinator
Accountable to: PCP Chief Executive & Board of Trustees
Located: Hybrid working between Catalyst House in Stockton, home and with significant outreach work across the Tees Valley (mostly Stockton and South Tees).
Hours: 22.5 hours per week (hours negotiable although some early evening and weekend work will be required)
Term: Fixed until 30th September 2024
Job Purpose:
PCP is seeking an experienced delivery officer to support the delivery of a range of wellbeing projects in the Tees Valley which aim to support individuals to be more connected with their local communities. In the first instance, this project will be the Community Connect Social Prescribing service.
As part of our Community Connect delivery, the successful applicant will support a caseload of clients to access a range of local services, ensuring they access the right support, at the right time, in the right place.
You will provide personalised assessment and take a holistic, person-centred approach to creating action plans to improve the social connectivity of local Stockton residents who are identified as being disconnected or isolated.
Job Description:
1. To manage a caseload of clients who are identified as being socially disconnected and isolated and follow the agreed processes to support them to access local groups and services independently.
2. To provide personalised assessment, signposting, information and take a holistic and supportive approach to enable clients to take steps towards community engagement.
3. To facilitate the development and delivery of personal action/activity plans, and work with clients to maintain motivation and assess progress against their individualised action/activity plan.
4. To signpost or refer individual clients to a range of health improvement lifestyle intervention, primary and secondary care services and voluntary and community support.
5. To promote the service to a range of groups and agencies, including GPs and other health professionals, and keep information about local services, referral options and sources of support up to date to ensure effective pathways for clients.
6. To effectively keep records and on client outcomes to contribute to performance monitoring, evaluation of outcomes, feedback to referrers and report writing.
7. To attend meetings with commissioners, partners and stakeholders, and support the quality monitoring process for the service.
8. To refer complex cases to Specialist Services (Health & Social Care) or appropriate services for re-assessment.
General:

3. To assist marketing and engagement work and use creative techniques to gather views from the communities we support.
4. To actively take responsibility for your own Health & Safety and ensuring procedures are adhered to.
5. To collate appropriate monitoring and evaluation information to support the achievement of agreed targets and outcomes within the project or service.
6. To carry out all responsibilities in line with the organisation’s Equality & Diversity Policy.
7. To recruit, support, train and motivate volunteers as required.
8. To undertake any training and development deemed appropriate.
9. To undertake any such duties required by your Senior Manager or PCP Chief Executive.
