What You'll Do Conduct Occupational Assessments and Interventions, emphasizing relapse prevention, establishing roles and routines and promoting recovery. Play a pivotal role in Discharge Planning by conducting Assessments, coordinating Interventions and ensuring a smooth transition for Patients back into the community. Conduct O.T Assessments in home environments to identify O.T interventions and Equipment needs as part of Discharge Planning. Work with Larch Ward's forthcoming Home First service staffed with O.T and O.T Clinical Support Worker. The Home First service provides up to 6 weeks OT intervention to support earlier and/or safer discharge from the Ward.
