Job summary
An exciting and rare opportunity has arisen to recruit a Motor Neurone Disease nurse specialist into the regional MND care centre in a job share role to cover 17.5hrs per week.
The MND service is a 5-day service providing specialist care, early intervention, and support to those with a diagnosis of MND in the Durham, Tees, and North Yorkshire areas. The service is led by a forward-thinking MDT, with patients' needs at its heart. You will be an enthusiastic and highly motivated individual with evidence and experience of providing care and support to those living with MND, palliative care, or neurological conditions.
You must have the ability to assess patients and formulate holistic management plans. You will also have the skill and ability to identify changes in a patient's condition, liaising and referring to other services and agencies as appropriate.
The successful candidate will be fully supported in this role by a well-established team, with vast knowledge and expertise in the management of those living with MND. Professional development is highly encouraged, with both internal and external learning opportunities available.
The post requires you to travel to deliver services to patients, carers, and families in their own homes.
Main duties of the job
The post holder will have a specific role in the coordination of care for people living with MND in the local community. They will be based at the MND Care Centre hub but will primarily work in the community, attending the MND MDT clinic weekly. A key responsibility is to identify early clinical deterioration, which may need intervention from the wider MND MDT, specialist respiratory, or palliative care teams. The post holder will ensure the NICE guidelines for MND are implemented throughout the MND care centre catchment area.

Person Specification
Knowledge and skills (Essential):
* Knowledge and experience of managing patients with MND, neurological conditions, or palliative care.
* Awareness of strategic issues influencing MND management.
* Knowledge of NICE guidelines for MND management.
* Problem solving and assessment skills.
* Excellent communication and interpersonal skills.
* Ability to work autonomously and as part of a multidisciplinary team.
Desirable:
* Knowledge of national progress in the specialty area.
* Leadership experience.
* Advanced clinical and/or prescribing skills.
Qualifications and training (Essential):
* NMC Level One Registration
* Diploma or first degree in nursing
* Evidence of recent MND-specific training
Desirable:
* Postgraduate qualification in Neurosciences/MND
* Master's degree or working towards
* Non-medical prescribing or working towards
Experience (Essential):
* Experience in MND, neurology, or palliative care.
* Experience collaborating with multidisciplinary agencies.
Desirable:
* Senior post experience in MND.
* Providing counselling to patients.
