A fantastic opportunity has arisen for an enthusiastic and motivated individual to join the team in developing the Hospital at Home and Medicines case management services across Manchester.
We are looking for an experienced pharmacist to promote innovative ways of working and positively contribute to local residents remaining in their own homes. You should be confident working as part of a multidisciplinary team with excellent organisational and interpersonal skills as well as advanced clinical knowledge.
The successful candidate will become an integral part of the well-supported and award-winning CMOS team in providing an advanced patient-facing pharmaceutical service in working closely with pharmacy teams and other services across MFT.
You must be registered with the GPhC as a pharmacist, hold a post-graduate clinical diploma or equivalent, and have substantial hospital or primary care experience. Ideally, you will be an Independent Prescriber or at least working towards achieving this.
Main duties of the job
1. Lead and oversee the medicines optimisation support to Hospital at Home (H@H).
2. Supervise pharmacists and technicians working in H@H.
3. Work in the H@H multidisciplinary team to:
1. Provide better coordinated care to patients.
2. Reduce hospital re-admissions.
4. Deputise for lead pharmacists in the Community Medicines Optimisation Service where appropriate.
5. Support the development of good practice and safe medicines management across LCO and its partner organisations.
6. Provide highly specialised clinical pharmacy knowledge and medicines advice to LCO staff and partner organisations, including cost-effective prescribing and clinical guidelines for disease treatment.
7. Work as an Independent Prescriber in H@H.
8. Develop and implement medicines management policies and procedures to support safe and cost-effective use of medicines in line with local priorities, national directives, medicines legislation, and professional ethics.
9. Lead on new service developments and medicines optimisation projects.
10. Support the delivery of a medicines optimisation service to other Community Medicines Optimisation Service (CMOS) work-streams when required.
11. Support the delivery of our medicines training to LCO staff.
12. Participate in any new arrangements that may involve flexible working, e.g. extended hours or 7-day service.

* Member of General Pharmaceutical Council
* Non-medical prescriber
Qualifications
* Vocational Master’s Degree in Pharmacy or equivalent
* Pre-registration training and successful completion of relevant examination
* Relevant clinical diploma / post-graduate qualification (or working towards this qualification) or evidence of equivalent clinical experience.
Knowledge, Skills & Experience
* A significant amount of post qualification experience in a relevant role practicing as a pharmacist
* Demonstrable experience of supervising other staff
* Demonstrable commitment to CPD
* Experience of investigation of medicines related incidents
* Demonstrable experience of training and support of pharmacists/other staff groups
* Experience of providing clinical pharmacy services in hospital
* Experience and knowledge of developing guidelines /protocols
* Experience of developing and implementing change
* Experience of providing prescribing advice to general practice
* Experience of working with primary care prescribing systems (e.g. EMIS)
You must have appropriate UK professional registration.
