Job summary IOW NHS Trust Community Children's Nurse Team Leader Band 7 (£46,148 - £52,809) We are looking for an enthusiastic Children's Community Nurse that holds the CCN SPQ qualification and registration on the Specialist Community part of the NMC register to lead the CCN team for 1 year to cover maternity leave. You will lead the Children's Community Nursing Team providing high quality holistic, family centred nursing care for children with long-term and complex health care needs on the Isle of Wight. You will act as a nurse advisor to families caring for children with long-term illness/complex needs in the community setting and provide education and advice to community care providers and acute services, to enable them to meet the needs of the complex patient. You will co-ordinate discharge plans and care packages of children and young people with complex needs. You will have day-to-day service delivery responsibility for the respite service and ensure that relationships and own behaviour is managed in line with the Isle of Wight NHS Trust competencies. Main duties of the job The Community Children's Nurse Team Leader is responsible for the ongoing assessment of care needs and the development, implementation and evaluation of programmes of care, for the CCN caseload. The Children's Community Team Leader will lead the skilled nursing care to children, including those with terminal and palliative care needs in their own homes and other community settings. Improve the outcomes for children who require care in the community including those with continuing care and long-term complex health needs and give support to the family and co-ordinate their packages of care. The post holder will have the ability to work independently under own initiative with the support and guidance of the Matron for Children and Young People. This will involve supporting junior staff and parents to enable them to care for the child or young person at home. To provide teaching to children and their carers within the community/education and acute care settings. To provide education to schools, respite services, care packages and other professionals in the 'Team around the Child', taking responsibility for setting and maintaining competency frameworks for each child within the caseload.
