Halton OOH District Nursing service, are seeking a committed and enthusiastic individual to join their team as a community staff nurse, covering the evening service.
The post is 18 hours per week over 3 evenings, hours are 17:00 - 23:00.
Applicants must have excellent interpersonal and communication skills, have a patient-centred approach to care, be highly motivated and able to work autonomously as well as part of a team.
The district nursing service provides nursing care to the housebound population of Halton to patients in their own homes as well as residential care homes. We also provide a treatment room service for ambulant patients and provide a comprehensive range of clinical skills.
You will work under the direction of the District Nursing Sister delivering:
* Administration of IV medication/Medicines management
* Health Promotion
* End of life / supportive care
* Holistic assessments
* Patient-centered Care
* Complex wound management
Main duties of the job
The successful candidate will perform holistic assessments of patient nursing needs. Plan, assess, implement and evaluate care delivery according to changing health needs utilising prescribing skills.
Ensure practice is evidence-based through undertaking audits and critically appraising literature to achieve a high standard of care.
Liaise with the District Nurse team leader to ensure information sharing in relation to patient care, changes to care plans, and the continuity of care and reviewing of complex care packages.
Mentoring, teaching and assessing.
Liaise with all health care professionals and other agencies to ensure effective service delivery.
Involve patients and carers in the planning and delivery of care.
Maintain accurate records in accordance with NMC Standards.
Participate in the health needs assessment of the practice population to identify areas of unmet need.
Ability to work without direct supervision and to solve problems.
Work on a rota basis to facilitate weekend working across the district nursing teams.

Job responsibilities

Perform comprehensive assessment of patient nursing needs as the named nurse.
Plan, assess and evaluate programmes of care according to changing health needs under the direction of the District Nurse.
Maintain accurate records in accordance with NMC.
Establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations.
Competently perform clinical skills related to the post.
Be professionally responsible for a delegated caseload under the supervision of the District Nurse.
Assess health needs at individual, practice and community level.
Undertake chronic disease management reviews using appropriate templates.
Work without direct supervision.

Qualifications
* NMC registered adult nurse
Skills & Attributes
* Able to perform complex assessments, plan, implement and evaluate care packages.
* Wide range of clinical skills including:
* Experience of working within palliative care.
* Able to communicate sensitive/unpleasant information to patients, relatives and/or carers.
* Knowledge of professional and NHS issues relating to specialist knowledge.
* Ability to develop effective interpersonal relationships with colleagues across health and social care settings.
* Ability to communicate at a high level both verbally and in writing.
* Teaching and Assessing qualification.
* Audit and research.
* Supra pubic catheterisation.
* Tissue viability qualification.
* IV Therapy.
* Chronic disease management.
* Experience of working within palliative care.
Experience
* Previous community experience.

* The ability to travel independently across the Trust.
