Due to internal promotion, an exciting and rare opportunity has arisen for an enthusiastic clinical pharmacist to join our friendly pharmacy team at University Hospitals Coventry and Warwickshire NHS Trust. The successful pharmacist will be part of our highly valued and skilled Oncology/ Haematology and Palliative Care team and play a vital role in providing a specialised clinical service to patients.
You will be based at University Hospital Coventry, a PFI hospital opened in 2006 which is situated just off the junction of the M6 and M69.
Although based in this large Acute Trust you will have an overarching role to support and develop the pharmaceutical support for patients requiring palliative care across the locality. You will work closely with medical, pharmacy and nursing teams both in the acute trusts and in the community to improve the pharmaceutical care of these patients. You will act as a key member of the multidisciplinary team thereby impacting on day to day prescribing to promote best and safe practice and reducing clinical risk. In addition you will join the Palliative Care team in the Acute Trust, to provide direct pharmaceutical support for patients within their care.
Main duties of the job
The primary role of this specialist post is to lead on the management and provision of a comprehensive, high quality pharmacy service to the patients requiring palliative care and end of life services across Coventry and Warwickshire. Based at University Hospital Coventry and Warwickshire NHS Trust (UHCW), the post holder will have an overarching role to support the care of these patients across all settings including the other acute trusts in the network, local hospices and community.
This position requires you to have a clinical diploma or equivalent, possess excellent communication skills and be able to work as a member of a multi-disciplinary team whilst being able to manage your time effectively.

Job responsibilities
The post holder will:
1. Work closely with medical and nursing staff to improve the pharmaceutical care of patients requiring palliative care and end of life services.
2. Work with and support the Myton Hospice Palliative care pharmacist based at UHCW.
3. Support the Pharmacy team to improve knowledge of palliative care as a speciality and review patients referred to the Palliative Care team at UHCW.
4. Be responsible for all aspects of Medicines Management within palliative care and end of life services at UHCW NHS Trust and have an overarching medicines management role within palliative care across Coventry and Warwickshire.
5. Lead on the provision of education to healthcare professionals in the pharmaceutical aspects of palliative care and end of life services across the above care settings.
6. Develop policies, guidelines, conduct audits and perform research relating to the use of medicines within palliative care and end of life services.
7. Support the Quality, Innovation and Patient Safety aspects of Palliative Care at UHCW to ensure care is outstanding and continually improving.
Role-specific duties
1. Provide a comprehensive clinical pharmacy service for palliative care patients at UHCW NHS Trust, attending board rounds and multidisciplinary team meetings.
2. Support a comprehensive clinical pharmacy service for palliative and end of life care patients across Coventry and Warwickshire by acting as pharmacist representative on the C.A.S.T.L.E. (Care and Support Towards Lifes End) expert advisory group which represents clinical providers of palliative and end of life care across Coventry and Warwickshire.
3. Provide pharmacy representation and input into other relevant groups such as the ICS EOL working group, the West Midlands Specialist Palliative Care Audit and Guidelines Group (SPAGG) and the Central England Palliative Care Pharmacy Group.
4. Provide strategic guidance on clinical pharmacy issues relating to palliative and end of life care services and provide professional pharmacy leadership in this area of care.
5. Proactively influence safe and cost-effective drug therapy with other senior clinical staff within palliative care and end of life services. This will include performing new drug evaluations, financial reporting and development of policies.
6. Identify areas where improvements in both quality and cost effectiveness of pharmaceutical care and prescribing may be encouraged, and influence others to incorporate this for the benefit of the wider health economy.
7. Work with senior staff in palliative and end of life care services to review policies and procedures to ensure safe and effective handling of medication. Ensure policies and procedures comply with existing legal requirements, best practice, national and local guidelines and CQC Requirements. Undertake risk management and conduct audits relating to supply, storage and administration of medicines.
8. Lead, develop and deliver education and training in the pharmaceutical aspects of palliative care to other healthcare professionals. To encourage and cultivate an environment of clinical and practice-based research and audit. To contribute to current and future research and audit projects associated with the use of medicines within palliative care.
9. To practice as an expert practitioner, providing advice and consultancy to patients, healthcare professionals and other agencies relating to pharmaceutical aspects of palliative and end of life care.
