The post holder will work as part of the Public Health Nursing Team within either Health Visiting (0-5) delivering services within an integrated care pathway. Under the supervision of the Specialist Community Public Health Nurse (SCPHN) Health Visitor, the post holder will support the delivery of the Health Visiting Delivery Model dependent on the perceived, expressed or assessed needs & levels of vulnerability of children & young people The post holder will work with key partners and communities to deliver targeted interventions to improve the health and wellbeing of children, young people and families to address health inequalities, especially in those from disadvantaged groups.
