North HUB CMHTs based at Northcroft are looking to employ a qualified staff member to support and manage a caseload of 30 clients with various mental health and social needs. Although the clinical work will be delivered with full autonomy, the post holder will work with a multidisciplinary team to best support their clients with professional advice from other disciplines.
Apart from managing their caseload, they will actively support our established and hardworking Duty Team when working with individuals who are in crisis or in need of urgent interventions. Additional responsibilities will include coverage of the Depot Clinic and completion of initial assessments for service users referred to secondary care.
Main duties of the job
The post holder:
1. Will act as an independent practitioner within a defined community setting, while liaising with other professionals as necessary.
2. Will be responsible for the assessment, planning, implementation, and evaluation of nursing care within agreed parameters, and have a working knowledge of local and national policies and legislation governing current service provision and audit.
3. Will ensure effective communication pathways exist.
4. Will promote equality, diversity, and rights in accordance with good practice and legislation.
5. Will identify and contribute to the training needs necessary for providing quality evidence-based care as appropriate.
