You will conduct care assessments, capacity assessments, and facilitate support planning. Responsibilities include triaging concerns, handling urgent cases, participating in safeguarding enquiries, and working in tandem with health professionals, housing, and reablement services. You will advocate for people requiring help, manage resources effectively, chair meetings, and report decisions utilizing assessment frameworks. You will also mentor new social workers and contribute to service planning. Skills Professional social work degree or equivalent Knowledge of the Care Act and relevant legal frameworks Experience with adult social care and safeguarding Strong report writing and organizational skills Capability to manage complex caseloads and work autonomously Skills in risk assessment and prioritization Excellent communication and interpersonal skills Ability to work under pressure in a fast-paced environment Social Work England registration Enhanced DBS check
