Advanced Nurse Practitioner in General Practice
A new and exciting opportunity has arisen for 1 WTE Advanced Nurse Practitioner/s to work as part of our established team. You will join the growing multi-disciplinary team for the PCN which includes Social Prescribing Link Workers, Care Coordinators, Clinical Pharmacists, Pharmacy Technicians, Trainee Nurse Associates, First Contact Physiotherapists, Health and Wellbeing Coaches and Primary Mental Health Nurses.
Main Duties of the Job
In your role you will work as part of the Sedgefield North PCN Integrated Team and will be aligned to multiple practices across the PCN. Sedgefield North PCN offers an ANP overflow service throughout winter months supporting practices with winter pressures, we are planning for this to be a permanent service provided to all practices throughout the year. Enhanced Access is part of the Network Contract DES whereby there is Primary Care provision from 8am-8pm Monday- Friday and 9am-5pm on a Saturday; the successful candidate/s hours will be during these hours.

Job Responsibilities
In your role you will work as part of the Sedgefield North PCN Integrated Team and will be aligned to multiple practices across the PCN. You will be expected to demonstrate advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles. The focus of the role is the provision of urgent/acute care, both on-site and possibly through visiting patients at home. You will work as an autonomous practitioner, providing high quality and timely patient-centred care to the practices' patient population and to be responsible for the delivery of evidence-based clinical care and nursing services, within your scope of practice.
You will adhere to professional standards, lead by example and maintain a positive, collaborative working relationship with the practices' multidisciplinary healthcare team, as well as supporting the management team in the reviewing and implementation of clinical policy and procedure.
The post holder will have successfully completed either a Centre for Advancing Practice accredited programme or the ePortfolio supported Route are eligible for an Advanced digital badge.

Qualifications
* Registered first level nurse MSc or equivalent
* Mentor teaching qualification
* Clinical supervision training and experience
* Extended independent nurse
