At the Dementia and Older People's Mental Health Liaison service, no day is the same. Our service is based within the Royal Cornwall Hospital, Truro, but we outreach to our 8 local community hospitals and also provide community visits to support our patients and carers when they leave hospital. As a band 6 nurse you will be involved in staff supervision, QI, audit, teaching, and development of our service.

Main duties of the job
The post of Psychiatric Liaison Nurse is a specialised mental health nurse role specialising in older adult psychiatry liaison.
You will be required to have extensive knowledge and skills in caring for older people with mental health problems, risk assessment and think outside of the box to ensure safe discharge out of the acute setting.
You will have a knowledge of the Discharge to Assess pathway and other funding routes.
The post holder will utilise these clinical skills in ensuring the psychiatric liaison team provide high quality psychosocial care and treatment, facilitating a holistic approach to service delivery and discharge planning.
The term holistic in this context is of particular importance, as patient's physical care needs in addition to psychological and social care needs must all be carefully considered in the acute general hospital setting.

Person Specification
Education
* Current professional registration - NMC, HCPC
* Evidence of continued professional development
* Having the mentoring qualification
Skills and attitude
* Experience of working with people with dementia post qualification
* Previous teaching/training experience
* Previous experience of complex case management
Knowledge and ability
* Ability to present and deliver informal and formal training/education to a range of parties including carers
* Proven leadership, organisational, prioritising and team working skills
