Young people who are transitioning from high level support, need more assistance when moving out of foster or residential care and out in to the community. Changing environments can be daunting...The Enhanced Transitional Support programme (ETS) is a brand new project that has been set up in partnership with Neath Port Talbot local authority, to help make the transition easier and more effective.
You will provide specialist, enhanced housing and related transitional support to vulnerable young people, within 24 hour supported accommodation. Your role will be an important one; you'll be the person they can rely on for help, advice, guidance, emotional and practical support. (Please note, there will be NO personal care involved in this role, it is NOT a health care position).
As the Support Worker, you will be based in the local area and support a small caseload of young people. You will work with the project based Support Assistants and cover the weekend (Sat and Sun 9.30am to 5pm).
What will you do?
* Support vulnerable young people to reduce the likelihood of homelessness
* Assisting young people to become more independent and self-sufficient
* Provide day-to-day emotional and practical support
* Be a guide and a mentor to those who need it most
* Keep young people safe from harm by reducing risk
* Achieve great outcomes by encouraging positive behaviours
* Work with other organisations to provide the best level of support and advice
What do you need?
* Experience of working with vulnerable young people
* A background in Social Care, Support Work, Education or similar
* Good understanding of safeguarding procedures
* Strong health, safety and wellbeing knowledge gained in supportive environment
* An understanding of trauma, ACE's and mental health issues
* Knowledge of housing and homelessness would be helpful
* Willing and able to carry out a new DBS check and provide at least two employer references
* Own transport is essential

* Permanent contract
* Part time is Saturday and Sunday 9.30am to 5pm
* Hourly rate £13.14
* 25 days annual leave raising 1 day per year up to 32 days (pro-rata)
* Pension scheme after 6 months

* Shifts will be 9.30am to 5pm Sat and Sun for part time.
