Clinical Responsibilities
1. Participating in the undertaking of health needs reviews and health assessments to identify needs and plan care accordingly.
2. To identify, plan and manage risk of an individual's care needs, relating to physical health, behavioural and/or mental health.
3. To be responsible for assessing, planning and making recommendations for future care in partnership with patients, carers, and multi-professional agencies including case conferences for reviewing the packages of care and care co-coordinating as appropriate.
4. To liaise with Primary and Secondary care agencies to ensure better health outcomes for individuals.
5. To work within the Mental Capacity Act and evidence best interest decision-making.
6. To maintain effective and appropriate communication networks with members of the multi-disciplinary team, the independent sector homes, and all stakeholder agencies and organisations to ensure that the individual care needs can be met.
7. To contribute to multi-disciplinary team specialist assessments.
8. Ability to work as an autonomous practitioner, planning, prioritising, and organising workload according to the needs of the service.
9. To provide any relevant information and explanation to clients and carers at a level commensurate with their ability to understand using total communication.
10. To provide evidence-based advice and guidance to care staff on the management of health needs to patients and signpost to other services.
11. To act as a supervisor and/or assessor for students demonstrating high standards of practice and professional conduct.
12. To undertake health promotion/education whenever possible to all adults with LD and also to contribute to planned health promotion activities.
13. To work with all other agencies with regards to the safeguarding process.
Managerial Responsibilities
1. Line Management responsibilities for staff band 5 and below.
2. To be responsible for coordinating reviews as per guidelines.
3. Prioritise caseload to meet competing demands.
4. To be aware of and abide by the LWSW policies, standards, and Clinical Governance initiatives.
5. Present confidentiality and be aware of the Data Protection Act, access to health records and consent for assessment process.
6. Involvement in continual audit and review of services to determine quality.
7. Maintain safe record-keeping and confidential practice, in line with LWSW and professional guidance and be aware of the Data Protection Act, access to health records and consent for assessment process.
8. To ensure electronic record keeping policy is adhered to.
Professional Responsibilities
1. To maintain a high professional standard of conduct at all times in accordance with NMC/professional bodies code of professional conduct. Maintain an active status on NMC/professional bodies register.
2. To maintain own re-validation portfolio.
3. Assume responsibility for own professional development and personal knowledge. Undertake and contribute to research where appropriate.
4. Monitor own performance against agreed objectives through the process of appraisal and personal development plans and professional body regulations.
5. Maintain professional expertise by arranging and attending meetings, study days, and mandatory training and service training.
6. Plan and participate in the supervision and provision of experience for all nurse learners and other personnel as appropriate.
7. Participate in regular line management, caseload management, and clinical supervision.
8. To deliver appropriate training and subsequent support to other staff, students, and outside agencies when required.
Communications and Relationships
The post holder will be required to work within the MDT function of the CLDT and may need to be part of wider MDT groups relating to client need. The post holder will liaise between other health, social, and educational services in assessing, planning, and reviewing client care needs/service provision. The post holder will also work with other voluntary and secondary care providers.
