Radis Community Care has an opportunity for a Team Leader at Henffordd Gardens in Hereford.
About the role
As a Team Leader, you will support individuals with PMLD (Profound and Multiple Learning Disabilities) and Mild Learning Disabilities. Responsibilities may include manual handling, medication administration, personal care, food preparation, household tasks, supporting individuals to maintain contact with family and friends, accessing the community, and attending appointments. We provide support for daily living skills and socialisation.
We recognise each person as an individual and offer a person-centred, tailored service. The role aims to promote independence, choice, dignity, and respect by delivering high standards of support. We provide a safe and secure home environment where individuals can live independently with the necessary support.
Shifts
Include early, late, and alternate weekend shifts with on-call duties.

Ideally, candidates will have a qualification in health and social care or experience in supported living settings and/or with individuals with PMLD, though this is not essential.
