The Fracture Liaison Service Coordinator will form an integral part of the multidisciplinary team approach in caring for patients at risk or diagnosed with osteoporosis or who have fallen and/or fractured. They will work in close unison with the Clinical Nurse Specialists/Consultants to ensure the coordination and management of patients at all points of their treatment in line with national guidelines.
The post holder will be expected to routinely monitor and validate the waiting times, ensure patients are booked accordingly and produce clinic letters following their appointments ensuring all clinical correspondence are sent out within a timely manner. Data collection is an important part of this role and integral to showing its success.
The Fracture Liaison Service Coordinator will be a point of contact fielding queries or escalating concerns with regards to the patient pathway.
1. To work with Consultants and Nurse Specialists across Rheumatology and Orthogeriatric team to
provide administrative support to the Fracture Liaison Service.
2. Coordinate patient referrals to the Fracture Liaison Service appropriately directing for triage and
ensure that all patients have an outcome following referral.
3. To check timing and process follow-up appointments, including ensuring blood tests are up to date.
To monitor patients through the patient pathway to ensure that care is provided in an efficient and
timely fashion and in accordance with treatment guidelines.
4. To exchange information with patients relating to appointments. This will include re-arranging
appointments and notifying patients of any cancellations that may occur.
