Band 6 Community nurse required for the out of hours service covering the Stockport Community. You will be visiting patients in their own homes including residential homes at night providing unplanned palliative care, End of life support, catheter care, symptom control and any unscheduled nursing need required by the Stockport Community to help prevent a hospital admission. You will be a community nurse with a keen interest in Palliative care and working out of hours. You will be working within a very knowledgeable welcoming team of nurses and Health care assistants with a robust preceptorship period. You will be liaising with out of hours Medical team Master call as an when required acting as an advocate for patients and their families/Carers whilst also Liaising with the District nursing day teams. You will be supporting the band 7 team lead with the day to day running of the team, alongside the overnight band 6 post holders. This role includes staff management with training, appraisals and rota.
